This article, derived from a master’s research in the field of Education, aims to understand the educational transition to higher education of a student with Autism Spectrum Disorder, based on the design, implementation, and monitoring of an Individualized Transition Plan. This qualitative study is grounded in the assumptions of action research and was designed as a single-subject case study. The primary participant was a student with Autism Spectrum Disorder, with the participation of two secondary participants: the student’s mother and the psychopedagogue responsible for his educational support. Data collection involved transition assessment and planning instruments, an interview with the student’s mother, and field diary records. The implementation of the Individualized Transition Plan took place in a virtual environment, using digital technologies and tools. The results indicate that the Individualized Transition Plan enhanced the student’s understanding of admission processes, academic demands, and support services, culminating in his admission to the Chemistry program at the Instituto Federal do Rio de Janeiro. The study also highlights the need for public policies to institutionalize the Individualized Transition Plan in the brazilian context.
